Directions

From I-96, take the E. Beltline exit and head North. Pass through the light at Leonard St. and take the 3rd 'Michigan turn' to head South on the E. Beltline. Smiles by Design is on the West(right-hand) side of the road in the 'Health Point Medical Building' at 1525 E. Beltline. We are right next door to 'Famous Dave's' BBQ restaurant. Suite 202 is on the second floor, directly across from the elevators.

Appointments

We know you have many choices when choosing a Dentist in Grand Rapids , MI so we have made requesting an appointment a simple process via our Web site. If, for any reason you cannot keep a scheduled appointment, or will be delayed, please call us as soon as possible.

Scheduling Details

We know your time is valuable. That is why we make our best effort to schedule your appointment at a time that is most convenient for you. Dr. Boone's time is valuable also. If you are unable to keep your appointment kindly give 48 hours notice. Failure to do so will result in a minimum $35 missed appointment fee.

Insurance and Billing

Smiles by Design accepts patients with or without Dental Insurance. We accept most dental insurance plans, however we do not accept Medicaid. Contact our office to verify acceptance of your plan. If you have an insurance plan, we will be happy to bill your insurance carrier for any procedures performed in our office. However, any fees charged by our office that are not covered by your insurance carrier will become the patient/guardian responsiblity.

Payment Options

We accept patient payment in the form of cash, check or Visa/Mastercard. We also offer an array of third-party patient financing options, which may open the doors to 0% Interest for up to 18 months or more for qualifying patients!
